Complete documentation helps avoid delays and ensures compliance with reimbursement standards. Key elements in the patient’s medical record include:
- A signed telehealth consent form
- Visit type (audio-only or audio and video)
- Start and stop times or total duration of the visit
- Patient and provider locations (e.g., home, office)
- Names of all participants involved in the telehealth session
In addition, please include the appropriate telehealth modifier or place of service code on all claims.
