You now can make authorization requests for TMS by phone
As of June 1, 2019, in addition to faxing requests for transcranial magnetic stimulation (TMS) authorizations, you now have the option to call in your requests using the number on the member’s benefit card. We’ve implemented this change in support of CMS standards.
If you are requesting services via phone, be prepared to present complete clinical information so that our care managers can determine if the case meets the applicable clinical guidelines.
If you are requesting services via fax, please continue to use the Fax Cover Sheet and the TMS Treatment Checklist. If you need copies of the cover sheet and checklist, contact Magellan from the phone number on the member’s benefit card. Complete both forms and fax to 1-888-656-4967.
- For initial requests, fill out the fax cover sheet and TMS checklist completely.
- For concurrent requests, fill out the fax cover sheet completely. And, on the TMS checklist, complete sections 3, 4 and 5, and include updated scores in comments section 6.
