Important: Use this new email address when requesting ABA services
New process for Blue Shield of California ABA requests
- Effective immediately, for Blue Shield of California applied behavior analysis (ABA) initial authorization and treatment plan / concurrent review requests, email forms to BSCABARequest@MagellanHealth.com.
- When sending records via email, ensure your message is encrypted. Check with your email service provider for more information about encryption/sending secure emails.
- The ABA prior authorization fax line (1-888-656-5409) will be discontinued.

You can find initial authorization and treatment plan / concurrent review forms online at MagellanProvider.com/Autism (requires sign in).
